Dental Procedure Consent

Please complete all fields below prior to your pet’s dental appointment. Our veterinary team will use this information to guide treatment decisions and ensure you’re kept informed of any findings or recommendations along the way.

In an effort to provide the best possible service for you and your pet, please take a moment to review and complete this admission form.

General cleaning includes: Pre-anaesthetic exam, I.V. catheter with fluids, general anaesthesia with monitoring, dental cleaning, scaling, polishing, charting, and full mouth radiographs (X-rays).

Pre-Anaesthetic Bloodwork

Extractions/Other Treatments:

Dental X-rays allow us to determine if any disease is occurring below the gum line. Severely diseased teeth can cause considerable pain and discomfort and are a source of infection for other organ systems (liver, kidney, lungs, and heart).
(if this box is not checked, a nail trim will be performed under anaesthesia)
Vomiting, diarrhea, seizures, etc.
